In the event of inpatient treatment;

  • Detailed hospital invoices, a report stating justification for admittance.
  • Detailed surgery report for surgical operations (including the pathology report, if a sample is taken).
  • Observation file, traffic accident minutes, judicial report, judicial minutes, alcohol report, insured’s statement, where required.
  • Epicrisis (procedure summary) report.
  • Where necessary, laparoscopic/arthroscopic/endoscopic operation record.
  • Any analysis and radiology reports regarding the inpatient treatment.

In the event of outpatient treatment;

For medical examinations:

  • Invoice or self-employment receipt stating the fee for the doctor
  • (the Doctor’s stamp and specialization must be included, cash register receipts are not accepted).
  • Original copy or report for any ultrasound procedure applied during the examination (where necessary, with medical records).

For medicine expenses:

  • The original copy of the doctor’s prescription (where necessary, doctor’s report).
  • Cash register receipt or invoice.
  • Medicine bar codes and box portions stating the name and price of the medicine.
  • Doctor’s report for medicine used on a continuous basis.

For diagnoses, roentgens and modern diagnostic techniques:

  • Doctor’s prescription/referral or report.
  • Invoices attesting relevant expenses.
  • Review results, reports and medical records where necessary.

For physical therapy:

  • Imaging results constituting the basis of treatment (MRI, Ultrasound, etc.).
  • Doctor’s prescription; detailed report describing the treatment stipulated (it is necessary to state the required treatment for each session and overall session count).

For maternity coverage:

  • Relevant birth and doctor’s report.
  • Detailed hospital invoice.
  • Where necessary, observation file.
  • In the case of compulsory abortion the gynecologic USG report, results of pathology or Beta HCG analysis should be submitted to the firm.

* Notarized translations of any foreign language documents required for claims payments, with reference to medical expenses incurred abroad, should be submitted to our firm.

Partial damages

  • Photocopy of the policy
  • Accident report (photos of the incident site should be taken in case new report type is used)
  • Witness statements and eye-witness report, if any
  • Detailed statement if no report is issued
  • Photocopies of the licenses of the vehicles involved in the accident
  • Photocopies of the driver’s licenses and alcohol examination reports
  • Insured’s bank account details
  • Republic of Turkey ID No. (of the insured and the driver)

Theft Claims

If the vehicle cannot be found within 30 days of theft;

  • Original copy of the police application report
  • Original copy of the police headquarters’ letter attesting inability to find the vehicle (the petition to request the letter will be provided by our firm)
  • Original copy of the Car Theft Department letter attesting inability to find the vehicle (the petition to request the letter will be provided by our firm)
  • Original copy of the “tax clearance certificate” regarding the vehicle
  • Original copy of the vehicle registration bearing “stolen” stamp
  • Original copies of the policy and receipts
  • Keys of the vehicle (original and spare)
  • Deed of consent to be obtained from the relevant entity in case of a loss payee
  • Power of Attorney (the model of the power of attorney will be provided by our firm)
  • Civil registry ID photocopy in case of private individuals
  • Tax chart, trade registry gazette and certified resolution for signatory authority
  • Vehicle file

Radio player deck theft claims

  • Police application report
  • Witness report
  • Photocopy of the license

Windshield claims

  • Accident/statement report or declaration
  • Photocopy of the vehicle’s license
  • Photocopy of the driver’s license in case the damage occurred while driving

* In addition to the documents listed above, different documents may be required as well with reference to the specifics of the incident.

Individuals who are involved in a traffic accident or who suffer material or bodily harm in the accident may submit applications directly with the Head Office or Regional Directorates of our firm, without the intermediation of a person or an entity, and file damage claims. All they need to do for this purpose is to obtain the following documents and submit them to our firm.

Accidents Involving Material Damage

  • Accident report (photos of the incident site should be taken in case new report type is used)
  • Witness statements and eye-witness report, if any
  • Photocopies of the licenses of the vehicles involved in the accident
  • Photocopies of the driver’s licenses and alcohol examination reports
  • Republic of Turkey ID Numbers of the drivers and vehicle owners
  • Details of the injured party vehicle owner’s bank account

Accidents resulting in death

  • Accident reports (witness statements and eye-witness report, if any)
  • Photocopies of the licenses of the vehicles involved in the accident
  • Photocopies of the driver’s licenses and alcohol examination reports
  • Certificate of inheritance
  • Certified copy of civil registration
  • Autopsy report
  • Official income certificate of the deceased

Accidents resulting in permanent disability

  • Accident reports (witness statements and eye-witness report, if any)
  • Photocopies of the licenses of the vehicles involved in the accident
  • Photocopies of the driver’s licenses and alcohol examination reports
  • Doctor’s reports regarding the treatment the injured person received after the accident
  • Medical board report attesting the level of final and permanent disability
  • Official income certificate stating the injured person’s income level before and after the accident

  • Original or photocopy of the policy

  • Insured’s statement regarding the occurrence of the damage

  • Non-prosecution decision (the public prosecutor’s bill of indictment if the decision will take a while to be issued)
  • Photocopy of the title deed and/or contract (in case of buildings)

  • Police Headquarters Report

In case of maritime shipping;

  • Original or photocopy of the policy
  • Original or photocopy of the invoice
  • Original or photocopy of the bill of lading
  • Vessel Reserve Report if the commodity was unloaded in a damaged form from the vessel (the report must bear the signature of the captain or agent of the vessel)
  • Examination Report
  • Customs Reserve Report if the commodity was damaged in a customs zone
  • The commodity should not be cleared at customs if it was damaged in a customs zone while the Customs Administration refrains from issuing a reserve report. Instead, an application should be filed with the Civil and Commercial Courts of First Instance of the place, asking for an official declaratory ruling. (The costs regarding the declaratory ruling shall be covered by the insurer)
  • The shipping contract if the shipping business was awarded on the basis of a contract
  • Cargo report (this document will not be required if the vessel reserve report was obtained instead)
  • Affreighter’s and its Turkish agent’s name/title and addresses
  • Navigation and packing list
  • Customs closing declaration
  • Tally and weight slips in sous palan discharges
  • Detached order or individual delivery order in case the vessel’s contents are short of the manifest, or lacking completely
  • Lloyd’s surveyor’s report for overseas claims
  • Loading manifest
  • Ullage Reports (oversight reports) for loading and discharge ports for liquid freights; preparation letter, empty tank certificate, protest letters

In Land/Air/Railway Shipments;

  • Policy, invoice, bill of lading or airway bill (delivery note in case of domestic shipments)
  • Reserve note in case the commodity was discharged in a damaged state (the note should contain the signatures of the relevant carriers, truck driver and railway administration)
  • Examination Report
  • Customs reserve note if the commodity was damaged in customs (the note should contain the signatures of customs officers or operators)
  • Court findings if customs refrains from issuing a Customs Administration reserve note even though the goods were damaged at customs
  • Detached order or individual delivery order if the vessel’s contents are short of the manifest, or lacking completely
  • Customs closing declaration
  • The shipping contract if the shipping was awarded on the basis of a contract
  • Traffic report and photocopies of the driver’s license and vehicle license if the damage was caused through an accident
  • The name, title and address of the carrier, and the name, title and address of its agency in Turkey
  • If the damage was not discovered during the discharge from the vehicle or at the first sight, or if the damage was discovered at a later time, for transportation operations with any vehicle, the carrier or its representative (agent) in the port of destination should be notified in written form within a maximum of three days.

  • Original or photocopy of the policy

For Construction/Assembly insurances

  • Police headquarters report for theft and accounting records (invoice, delivery note, etc.) for insured assets
